At Jefferson House we provide excellent long-term care and rehabilitation services to seniors in the greater Hartford area for over 120 years. We are comprised of a skilled and caring staff which provides seniors with Nursing Services, Therapeutic Recreation, Spiritual Services, and Social Services, aimed at caring for the whole individual.
Monitors and maintains a focus on patient outcomes for continuous quality assurance and performance improvement. Supports community wide staff education in areas that are linked to quality assurance and performance improvement (QAPI) across all departments. Conducts educational sessions focused on improving clinical outcomes. Focuses on both patient and process outcomes for continuous quality improvement. Designs and monitors indicators using SCC’s QAPI process to generate accurate and timely reports. Reviews all incident reports and exercises professional judgment in assuring patient confidentiality as a high priority. Demonstrates competencies to meet age specific patient and resident needs as evidenced by positive outcomes with focus on customer service, culture change and documentation.
