Physical Therapist (PT) - Skilled Nursing Facility - Lowell, MA
Seeking a traveling Physical Therapist to provide evaluation, treatment, and rehabilitation services in a skilled nursing setting. The therapist will screen, evaluate, and treat patients with physical functioning disorders and wounds, develop and implement plans of care, utilize standardized assessments and evidence-based interventions, and document outcomes per regulatory and clinical requirements. The role includes patient and family education, interdisciplinary communication, participation in care conferences and facility-required trainings, and supervision of aides, assistants, or students as needed.
Minimum Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active MA state Physical Therapy license is required to start the assignment.
- Minimum 1 year of PT experience; LTC or rehabilitation experience required.
- New graduates are considered if they have Level II clinical experience rather than only Level I observation experience.
- Ability to perform standardized assessments, manual techniques, therapeutic exercise, ambulation training, modalities, use of assistive devices, and wound care per facility standards.
- Document clinical care accurately in electronic medical records and record treatment minutes per regulatory requirements.
- Ability to participate in interdisciplinary meetings and required facility trainings.
- BLS certification may be required from AHA or ARC.
Preferred Qualifications
- Experience supervising physical therapist assistants, rehabilitation aides, or students.
- Familiarity with regulatory requirements for skilled nursing and long-term care documentation and outcomes measurement.
Assignment Details
- Shift schedule: 8 hour days.
- Weekend requirement: None.
- Must be able to obtain necessary DME or supplies per clinical requirements.
