Minimum Qualifications
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent entry-level degree in Physical Therapy required.
- Current state license as a Physical Therapist in Florida required to start the assignment.
- Proof of current CPR certification required.
- Current driver’s license, reliable transportation, and insurance required.
- 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting required.
- Ability to perform heavy lifting, bending, pulling, pushing, prolonged standing and walking.
- Ability to work a flexible schedule and travel locally.
- Must read, write, and speak fluent English.
- Must meet mandatory continuing education requirements.
Preferred Qualifications
- Previous home care experience preferred.

- Must be willing and able to travel locally as part of job duties.

Impacting Patient Care
Provide assessment, plan of care development, therapeutic treatments, patient and family education, and documentation for a diverse home health population. Collaborate with interdisciplinary teams and follow infection control and universal precautions. Supervise other team members when applicable. Maintain documentation including POCs, progress notes, and phone orders in accordance with agency and regulatory requirements.
