Minimum Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Kentucky Physical Therapist license required to start the assignment.
- Home health experience required.
- Experience with OASIS and HCHB required.
- BLS certification from AHA or ARC may be required.
- OASIS certification preferred.
- Must be able to complete SOCs/ROCs, routine visits, supervisory visits, and re-assessments.

Assignment Details
- Shift hours: Monday–Friday, 8:00 AM–4:30 PM.
- Weekly guaranteed hours: 32.
- On-call or weekend requirement: No.
- Will be reimbursed for mileage between visits from first patient to last patient.
- Coverage areas include Shelby County, eastern Jefferson County, and Spencer County.
