Certified Wellness & Activity Director (CNA) at Stone Oak Care Center
Stone Oak Care Center is seeking a Certified Wellness Director (CNA) who is passionate about Making Lives Better by creating engaging, meaningful programs for residents and promoting a vibrant, connected community culture. This role plays a key part in delivering The Touchstone Experience—where purpose, compassion, and purpose come together.
Key Responsibilities
- Plan, schedule, and lead individual and group wellness activities based on resident assessments and interests.
- Create and maintain engaging monthly activity calendars reflecting resident preferences and community events.
- Complete assigned MDS sections accurately and timely.
- Utilize RAPs to determine activity-related care planning needs.
- Develop, document, and update individualized activity care plans (reviewed at least quarterly).
- Document resident and family interactions per state and federal guidelines.
- Serve as an active member of the interdisciplinary team.
- Plan and assist with Residents’ Council meetings.
- Support a positive, energetic environment for residents and team members.
Qualifications
- Current Certified Activity Director certification (required)
- CNA certification in good standing and valid in the state of Texas
- Prior Wellness/Activities Director experience in a SNF preferred
- Strong organizational, communication, and engagement skills
