Minimum Requirements
- Doctoral, Master’s, or Bachelor’s Degree in Physical Therapy from an accredited program approved by the APTA.
- Active West Virginia Physical Therapist license required prior to start.
- Home health experience required. Experience providing patient care in home settings preferred.
- Familiarity with HCHB EMR preferred.
- BLS certification from AHA or ARC may be required.
Assignment Details
- Facility type: Home Health.
- Shift name: Days.
- Shift details: Monday–Friday, 0800–1600 (5x8).
- Weekly guaranteed hours: 40.
- Productivity expectation: 30 points per week.
- Coverage area: Kingwood, WV and surrounding two counties. Mileage reimbursed between visits from first patient to last patient.
- On-call/weekend requirement: No.
