Minimum Requirements
- Doctoral, Master’s, or Bachelor’s Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Illinois Physical Therapist license is required to start the assignment. Pending state license accepted.
- BLS certification from AHA or ARC required.
- Minimum 1 year of clinical experience in physical therapy. Home health experience preferred.
- Experience with orthopedic, cardiac, neurologic, pulmonary, and diabetes patient populations preferred.
- Case management skills, ability to work independently, time management and organizational skills required.
- Ability to supervise PTA and function as part of a multidisciplinary team.
- Locals accepted.
Assignment Details
- Facility Type: Home Health.
- Shift Name: Days.
- Shift Details: 5x8 schedule, Monday–Friday, approximately 08:00–16:30.
- Typical caseload: 5–6 units per day.
- Patient population: adults and geriatrics only; no pediatric patients.
- Weekend requirement: every other weekend required per client policy.
- On-call: No.
- Floating: No floating to other facilities required.
