Minimum Requirements
- Associate's, Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
- Active Pennsylvania Physical Therapy license required to start the assignment.
- Home Health experience required.
- OASIS assessment experience required.
- Reliable vehicle required for patient visits.
- BLS certification from AHA or ARC may be required.
- HCHB documentation experience preferred.
- Experience supervising physical therapy assistants preferred.
Assignment Details
- Facility Type: Home Health.
- Shift Name: Days.
- Shift Details: Monday–Friday, 08:00–16:00 (daily schedule includes OASIS assessments, plan of care development, and in-home PT treatment).
- Weekly hours: 36 hours guarantee.
- Coverage Areas: Clearfield County and Centre County; travel between patient homes required. Mileage reimbursement provided at $0.50/mile from first to last patient visit.
- On-call/Weekend Requirement: none.
