Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Florida Physical Therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Must have at least 12 months of Home Health experience within the past 3 years (client requirement).
- Experience with HCHB EMR is required (client requirement).
- Ability to cover a territory that includes Bay County and surrounding areas, including Panama City and Panama City Beach (client requirement).
- Must be able to complete required documentation and deliver care consistent with home health regulations and agency policies.
- Preferred: previous travel/home health physical therapy experience beyond the minimum requirement.
Assignment Details
- Facility Type: Home Health.
- Shift Name: Days.
- Shift Schedule: Monday through Friday, 8:00 AM to 5:00 PM.
- Shift Type: 8-hour days.
- Guaranteed hours: 40 hours per week and 30 patient visit points per week (client-provided guarantee).
- Length of Assignment: 13 weeks.
- On call: No.
- Territory: Bay County and surrounding areas including Panama City and Panama City Beach; travel within the territory required.
