Minimum Requirements
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program (required).
- Current State License as a Physical Therapist in Kentucky or ability to obtain prior to start (required).
- Proof of current CPR certification (required).
- Current driver's license, reliable transportation, and vehicle insurance (required).
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting (required).
- Previous home health experience (preferred).
- Ability to perform prolonged standing, walking, lifting, bending, pushing, and pulling as required for in-home care (required).
- Ability to work a flexible schedule and travel locally (required).
- Demonstrated knowledge of durable medical equipment and ability to order/adapt equipment for home environments (required).
- Ability to read, write, and speak fluent English (required).
Assignment Details

- Must provide patient evaluations, develop and revise Plans of Care, document progress notes, and process phone orders.

- Must be willing to perform job duties consistent with education and competence and accept assigned caseloads in the home health setting.
