Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Georgia Physical Therapist license required prior to the start of the assignment.
- BLS certification from American Heart Association or American Red Cross may be required.
- Home health experience preferred.
- Experience with HCHB (HomeCare HomeBase) preferred.
- Orthopedic patient experience preferred.
- Ability to complete comprehensive PT evaluations, develop measurable goals, and document per company and regulatory requirements.
- Ability to supervise PT Assistants and PT Aides in accordance with state practice act.
- Willingness to cover patients across Henry, Spalding, and Pike counties as assigned.
- Productivity expectation: 30 points (client requirement).
