Minimum Requirements
- Bachelor’s, Master’s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active California Physical Therapist license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Home health experience required.
- Experience with HCHB (Homecare HomeBase) required.
- Productivity expectation: ability to meet approximately 5 productivity points per day (can be combination of follow-up visits and Start of Care visits).
- Previous experience managing territory assignments or caseloads across multiple teams is preferred.

- Facility Type: Home Health.
- Shift Name: Days.
- Shift Details: Days, 8-hour shifts, Monday through Friday.
- Typical Schedule: Monday to Friday, 8:00 AM to 5:00 PM.
- Assignment Length: 12 weeks.
- On-call or weekend requirements: None.
- Territory: Responsible for visits within assigned teams (multiple teams).
