Minimum Requirements
- Associate's, Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Preferred: Bachelor's, Master's, or Doctoral degree.
- Active Florida Physical Therapy license required to start the assignment. Preferred: Florida-licensed Physical Therapist (not required if license transferable by start date).
- OASIS proficiency required. Client requires OASIS-experienced clinicians.
- Experience in Home Health settings required, including travel to patient homes daily across a multi-site service area.
- Minimum education requirement: Associate degree required by client; higher degrees accepted.
- BLS certification from AHA or ARC may be required.
- Ability to complete required documentation and perform six patient visits per day as part of routine caseload management.
- Preferred: experience with electronic documentation platforms and mobility assessment in home environments.
Assignment Details
- Facility type: Home Health.
- Shift Name: Evenings.
- Shift Details: Evenings 2000-0400.
- Work days: Monday to Friday as described by client.
- Daily travel required to patient homes throughout the Gulf Breeze to Destin area. Travel between patient locations is required each shift.
- Caseload expectation: perform approximately six patient visits per day.
- Dress code: royal blue scrubs or docker pants with polo-type shirt.
