Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by APTA.
- Current, active, and unencumbered Physical Therapist license for the state of assignment (Arizona license required if assigned to AZ).
- Current BLS/CPR certification from AHA or ARC.
- Home health experience preferred.
- Experience providing evaluations, developing and updating plans of care, and documenting patient progress in accordance with physician orders.
- Experience supervising Physical Therapist Assistants and Home Health Aides preferred.
- Reliable transportation, valid driver’s license, and agency-required liability insurance as required for home visits.
- Basic computer proficiency including email, electronic documentation, and online training systems.
- Two to three years of physical therapy experience preferred.
