Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Ohio Physical Therapist license required to start the assignment.
- BLS certification from the American Heart Association or American Red Cross may be required.
- Minimum 12 months of home health experience within the past 3 years (client requirement).
- Experience with HCHB and OASIS documentation systems (client requirement).
- Ability to meet productivity expectations of 30 points per week and maintain a 40 hours per week schedule as required by the client.
- Reliable transportation and ability to perform home visits in the local service area.
