Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active West Virginia Physical Therapist license required to start the assignment.
- BLS (Basic Life Support) certification from the American Heart Association or American Red Cross may be required.
- Home health experience required. Experience with OASIS assessments required.
- Experience using home care documentation software preferred. HCHB experience preferred.
- Ability to perform comprehensive PT evaluations, establish measurable goals, develop and modify treatment plans, and document according to facility, payer, and regulatory requirements.
- Experience supervising PT assistants and aides in accordance with state practice act preferred.

- Facility Type: Home Health.
- Shift: Days. Monday through Friday, 8:00 AM–5:00 PM.
- Job Type: Travel contract.
- County coverage area includes Monongalia, Preston, Wetzel, Tucker, and Grant counties.
- No on-call or weekend requirements.
