Minimum Requirements
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active New Jersey State Physical Therapist license required to start the assignment.
- BLS (Basic Life Support) certification from AHA or ARC required or must be obtained prior to start.
- Minimum of 1 year of recent professional SNF (Skilled Nursing Facility) experience required. This includes handling post-acute care caseloads, restorative programs, and working with interdisciplinary teams in long-term care settings.
- Experience with common SNF documentation systems and standards, including progress notes, care plans, and MDS-related communication preferred.
- Ability to manage typical SNF equipment and modalities, including gait training devices, therapeutic exercise equipment, fall prevention strategies, and safe patient transfers.
- Current CPR certification required.
- Must be able to pass facility background checks and any required health screenings.
- Preferred: experience with restorative nursing programs, wound care collaboration, and working with geriatric populations.
