Schedule
• Monday–Friday
• 8 a.m.–5 p.m.
• Only 2 holiday on-call shifts per year

What You’ll Do
• Provide direct patient care in accordance with the State Nurse Practice Act
• Complete comprehensive assessments and follow the individualized plan of care
• Document patient services accurately and in a timely manner
• Educate patients and families/caregivers about diagnoses and care plans
• Collaborate with the interdisciplinary team (aides, OT, PT, MSW, Dietitian, Clinical Supervisor)
• Manage pain and symptoms effectively and evaluate patient response
• Support patients and families with compassion, professionalism, and respect

What You Bring
• Graduate of an accredited practical or vocational nursing program
• Current LPN license in the state
• At least 1 year of medical/surgical experience (home health or hospice preferred)
• Current CPR certification
• Valid driver’s license and reliable, insured vehicle
