Minimum Requirements:
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active California Physical Therapy license required to start the position.
- BLS certification from AHA or ARC may be required.
- Home health experience preferred. SNF experience acceptable.
- Experience with WellSky Kinser EMR required.
- OASIS experience preferred. Candidates with home health and OASIS experience will be considered for relocation support.
- Ability to manage a full-time caseload of approximately 30 points per week (roughly 4–5 patients per day).
- Strong communication, teamwork, adaptability, and problem-solving skills.
- Must be able to begin seeing patients and document within expected timeframes; timely documentation (within 48 hours) is required.

- Setting: Home health, localized patient care in the San Jose, CA area.
- Shift: Days. Typical schedule M–F with flexibility. Scheduler can accommodate preferred hours.
- Reporting: Regional Therapy Director.
- Caseload Expectations: Full-time caseload, approximately 30 points/week (4–5 patients/day).
- EMR: WellSky Kinser required.
- Reason for opening: Growth in volume.
- Training: Candidate must be able to hit the ground running and work independently with minimal onboarding.
