What You’ll Do
• Provide personal care and support based on each patient’s individualized plan of care
• Assist with bathing, grooming, dressing, oral care, and other daily living needs
• Ensure patient safety and proper use of equipment
• Prepare nutritious meals and assist with feeding as assigned
• Take and record vital signs when ordered and competency is demonstrated
• Monitor skin integrity and help prevent breakdown
• Report changes in patient condition to the Case Manager
• Offer respite and emotional support to families and caregivers
What You Bring
• At least 16 years of age
• State-certified nurse aide
• Current CPR certification
• Clear listing on the state aide registry
• Understanding of hospice philosophy and comfort working with terminally ill patients
• Compassion, reliability, and strong communication skills
