Are you a dedicated Physical Therapist looking to make a meaningful impact in home health care? Do you have the expertise in HCHB and OASIS to provide exceptional care to patients in their homes? If so, we have an exciting opportunity for you to join our healthcare partner in Clearlake, CA.
In this role, you will work primarily day shifts from Monday to Friday, with the option to pick up additional shifts on weekends if desired. As a Physical Therapist, you will play a crucial role in helping patients regain their mobility and improve their quality of life.
- Provide comprehensive physical therapy services to patients in a home health setting
- Utilize HCHB and OASIS to document patient care accurately
- Collaborate with a multidisciplinary team to develop personalized care plans
- Travel to patient homes, ensuring timely and effective care delivery
- Maintain compliance with all relevant regulations and guidelines
Requirements:
- Doctor of Physical Therapy (DPT) degree (required if licensed on or after January 2016)
- Minimum of 2 years of experience in home health care
- Current CPR or Basic Life Support (BLS) certification
- Valid driver's license and must be 21 years of age or older
- Mileage reimbursement available at Government rate
