Minimum Requirements:
- Must have State License to Apply
- 6 months Pharmacist experience preferred
- EPIC experience
- Experience with Medication Reconciliation for discharge medication list
- Communication skills for gathering prior to admission home medication list from patient, family, or pharmacy
- Discharge counseling with patients/families
- Clinical knowledge for Antimicrobial Stewardship
- Knowledge of UTI, pneumonia, SSTI for recommendations
- Pharmacokinetic Dosing Management/Monitoring
- Anticoagulation Dosing Management/Monitoring
