Job Overview
The position is responsible for the provision of nursing assessments, interventions and supportive care. The scope of the on-call nurse includes telephone triage, direct patient care, crisis intervention, problem solving, patient/caregiver advocacy and support, with emphasis of avoiding hospitalization in the off hours. This position provides nursing coverage evenings/nights and on a rotating basis for weekends and holidays dependent on operational need.
Hours: 7 Days on 7 Days off (Nights)

Job Description
Minimum Qualifications:
1. Massachusetts RN Licensure.
2. New Hampshire RN Licensure.
3. Basic Life Support (BLS) certification.
4. Five (5) years of medical-surgical experience including one (1) year in one of the following areas: oncology, hospice, or home health.
5. Valid driver’s license
Preferred Qualifications:
1. Bachelor’s degree in nursing
Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.
1.Ensures continuous coverage for incoming calls after office hours and weekends, therefore, is available by telephone to patients/caregivers, physicians and staff.
2.Promptly triages patient/caregiver related phone calls to assess need for visit and ensures that necessary visits are rendered in a timely manner.
3.Appropriately triages patient visits according to priority and urgency.
4.Ability to manage time effectively, prioritize work when schedule is busy.
5.Assesses patients’ physical, psychosocial needs in a sensitive, caring manner following established Standards of Nursing Practice and Agency policies and procedures.
6.Demonstrates competence in Home Health and Hospice standards of care, symptom management, psychosocial, and spiritual aspects of care.
7.Maintains a current body of knowledge and clinical competence through ongoing participation in continuing education relevant to the area of practice.
8.Possesses communication skills needed to effectively understand patient’s reactions or responses to treatment.
9.Demonstrates ability to function independently utilizing excellent assessment /decision making skills.
10.Recognizes/identifies patients/caregivers cultural beliefs re: death and dying and is sensitive to those beliefs.
11.Collaborates with Clinical Managers and RN Case Managers for problem solving and as a resource.
12.Documents patient data in patient record using computer system in a timely, organized, concise manner to ensure compliance with legal, certification, third party payor guidelines and Agency policies.
13.Communicates unusual events, problems, or concerns to appropriate Clinical Manager/Director.
14.Notifies primary clinicians/ Clinical Manager of patient status and need for subsequent visits.
15.Conducts all care activities with respect for a patient’s privacy and rights.
16.Documents all patient/caregiver telephone calls and visits as to reason and nursing action/intervention in EMR.
17.Demonstrates ability to cope with patient/family emotional stress and provide appropriate supportive care.
18.Upon patient death, adheres to Agency Pronouncement Policy.
19.Seeks consultation with Clinical Manager/ Director with unusual/unfamiliar patient care situations and/or questions re: Agency policies/ procedures.
20.Participates in holiday coverage per Agency policy.
21.Demonstrates and practices Agency safety guidelines and procedures including general ergonomics, fire, and personal safety. Utilizes proper body mechanics and patient material handling.
22.Utilizes proper infection control practices per Agency policy and OSHA guidelines.
23.Performs other duties as requested by Clinical Managers/Directors.
24.Demonstrates a knowledge and understanding of what to report to the CEO or Chief Compliance Officer when concerns of corporate compliance arise.
25.Ensures compliance within guidelines set forth by regulatory agencies (DPH, ERISA etc.) and demonstrates compliance with Home Health Foundation policies and procedures.
26.Practices confidentiality principles set by the agency and federal HIPAA/HITECH guidelines.
27.Participates in all mandatory in-services.
